You get paid per click on the page, and sometimes there is a maximum click amount per day, or minimum time between clicks on page, you get paid for. Actually you don't have to use the page as a startpage, but that will help you get more clicks. On some of the companies you have to download a tracker that counts your clicks (like ignifuge), on others you can just start make money (like paysu).
